To make this official, we need four things from you:

  1. Your W-9

  2. Proof of commercial general liability insurance (COI)* with Hammer & Nail, LLC listed as Additional Insured (your insurance agent can handle this!)

  3. Proof of workers’ comp if you have employees OR a workers’ comp waiver (talk to your insurance agent)

  4. A simple signed sub agreement that spells out how we work together (will be sent to you later)
